I understand and agree to allow West Texas Rehabilitation Center (WTRC) to use the information, photo, and/or video I am submitting. I acknowledge that the materials I provide may be used for marketing, public outreach, or other communications that promote the mission of West Texas Rehabilitation Center. I further understand that this content may be shared across various media platforms and formats, without geographic or time limitations.

I authorize the release of any information, including health-related information, contained in this submission, and I give permission for WTRC staff to discuss the details with physicians or other staff members if necessary to verify accuracy.

I waive and release West Texas Rehabilitation Center, its officers, directors, employees, agents, and affiliates from any and all claims related to the use or distribution of this content. I also waive any right to royalties or other compensation resulting from WTRC’s use of my submission.

I assign all rights, including copyright, to the submitted materials to West Texas Rehabilitation Center. I represent and warrant that I have the legal authority to share this content and that no other party holds ownership or superior rights to the materials.

I have read, understand, and voluntarily agree to be legally bound by this waiver, release, and authorization.
