General Liability Waiver

I hereby give my consent for myself and/or my child to participate in any and all activities prepared by West Texas Rehabilitation Center during the selected event or program. In exchange for the acceptance of participation, I assume all risks and hazards incidental to the conduct of the activities, and release, absolve, and hold harmless West Texas Rehabilitation Center, along with its officers, agents, and representatives, from any and all liability for injuries to myself and/or my child arising out of travel to, participation in, or return from the event or program.
In the event of injury, I hereby waive all claims against West Texas Rehabilitation Center, including all affiliates, participants, and, if applicable, the owners and lessors of premises used to conduct the event.
